According to tradition, this wooden chapel was built on the outskirts of Młodzów during the plague that struck the city in the 17th century. It was restored in 1895. The appearance of the current building indicates its construction or a thorough reconstruction at the turn of the 19th and 20th centuries. In the altar is a painting of St. Rozalia, painted by Stanisław Zarzecki in 1897.
